Clinical-psychological peculiarities of patients with depression and diff erent levels of medication compliance (diagnosis and correction)

Abstract

An analysis of medication compliance (MC) levels in patients with depressive disorders demonstrated lack of MC in 31.8 % of the patients, a partial MC in 36.4 % of the patients, and a complete MC in 31.7 % of patients. An investigation of MC levels in groups of patients with depressive episode and recurrent depression evidenced a preservation of the trends revealed: lack of MC was registered in 30.6 % and 33.0 %, a partial MC was registered in 36.9 % and 35.8 %, and a complete MC was registered in 32.4 % and 31.1 % correspondingly.

On the base of data obtained it was worked out and implemented an integrated system of therapy for depressive disorders accompanied by MC breaches. Measures of the system are directed to the main factors promoting breaches of MC (factors associated with disease, with personality of the patient, with medication, social-psychological and healthcare organizational factors).

An evaluation of clinical efficacy of the integrated system of measures (taking into account MC recovery) demonstrated that patients treated in accordance with the principles described, had recovery in 35.5 % (p < 0.05), an amelioration in 43.5 % (p < 0.05), and lack of dynamics in 21.0 % (p < 0.05). In the control group with therapy without taking into account of MC peculiarities the results of treatment were diff erent: recovery was registered in 21.3 %, an amelioration was registered in 30.8 %, lack of positive dynamics was registered in 33.8 %, and aggravation was registered in 13.8 %.
