POLYSUBSTANCE ALCOHOL DEPENDENCE IN THE CONTEXT OF WAR: CLINICAL MANIFESTATIONS, STRESS-RELATED COMORBIDITY, AND DIFFERENTIATED TREATMENT APPROACHES

Abstract

Mental and behavioural disorders due to the use of alcohol and other psychoactive substances remain a significant medical and social problem. In the context of war, they acquire additional clinical complexity due to the combined effects of chronic stress, psychological traumatization, sleep deprivation, pain, injuries, moral injury, and limited access to care. Particular attention should be paid to polysubstance patterns in which alcohol dependence is combined with the use of amphetamine, methamphetamine, or cannabis.

Aim. The aim of the article is to summarize current scientific evidence on the psychopathological, behavioral, somatic, and therapeutic profile of alcohol dependence in the context of comorbid use of other psychoactive substances under wartime conditions.

A narrative literature review was conducted, including an analysis of international clinical guidelines, systematic and narrative reviews, epidemiological studies, and analytical materials relevant to alcohol dependence, polysubstance use, stress-related disorders, and war-related psychological traumatization.

Alcohol dependence with comorbid use of amphetamine, methamphetamine, or cannabis in the context of war is a clinically complex polysubstance condition that requires the simultaneous assessment of dependence, stress-related psychopathology, suicidal and aggressive risk, sleep disturbances, somatic complications, and functional maladaptation.
