CLINICAL-PHARMACOLOGICAL COMPATIBILITY OF BUPRENORPHINE WITH ANTIVIRAL DRUGS FOR THE TREATMENT OF CHRONIC VIRAL HEPATITIS B AND D IN PATIENTS WITH OPIOID ADDICTION

Abstract

The aim of the study is to determine the clinical and pharmacological compatibility of buprenorphine with antiviral drugs in the treatment of chronic viral hepatitis B and D in patients with opioid dependence. The work used bibliosemantic, analytical and comparative methods with the analysis of evidence-based medicine data, as well as drug interaction protocols from the certified databases “DrugBank” and “Hep Drug Interactions”. It was found that nucleoside analogues (entecavir, tenofovir disoproxil fumarate, tenofovir alafenamide), lamivudine and bulevertide are pharmacokinetically compatible with buprenorphine and do not affect its metabolism through the CYP450 system. The combinations are assessed as safe and do not require routine dose adjustment. Peginterferons also do not demonstrate clinically significant interactions with buprenorphine. At the same time, an increase in methadone exposure has been described with peginterferons, which emphasizes the advantages of buprenorphine. Thus, buprenorphine has a favorable drug interaction profile and can be safely used as part of complex therapy of chronic viral hepatitis B and D in patients with opioid dependence. Integration of substitution therapy and antiviral treatment is clinically appropriate and requires an individualized approach and monitoring.
