ПСИХОЛОГІЧНІ МЕТОДИ ТЕРАПІЇ ХРОНІЧНОГО СКЕЛЕТНО-М’ЯЗОВОГО БОЛЮ: СУЧАСНИЙ ПОГЛЯД НА БІОПСИХОСОЦІАЛЬНУ МОДЕЛЬ ТА ДОКАЗОВІ ІНТЕРВЕНЦІЇ

Анотація

Хронічний скелетно-м’язовий біль (ХСМБ) є  однією з  найактуальніших медико-соціальних проблем, що  призводить до  інвалідизації та  зниження якості життя. З  огляду на  обмежену ефективність і  ризики традиційної фармакотерапії, особливого значення набувають психологічні методи як складник мультидисциплінарного ведення пацієнтів.

Мета дослідження: узагальнення та  систематизація сучасних наукових даних щодо психологічних методів терапії ХСМБ з урахуванням нейробіологічних досягнень, клінічних рекомендацій і  доказової бази.

Проведено аналіз публікацій у базах PubMed, Scopus, Web of Science та Google Scholar за 2015—2026 рр. Розглянуто діагностичні підходи, що включають виявлення «червоних прапорців», та психологічний скринінг за шкалами PCS, HADS, TSK, GSE, CSI. Підкреслено ключову роль терапевтичного альянсу. Проаналізовано когнітивно-поведінкову терапію (КПТ) як  «золотий стандарт» (рівень доказовості  А), а  також терапію прийняття та  відповідальності (ACT), практики усвідомленості (MBSR, MBCT), релаксацію, поведінкову активацію і  стратегії самоменеджменту.

Проведено порівняльний аналіз методів за  доказовістю, механізмами дії та  ефективністю. Запропоновано авторський алгоритм вибору інтервенцій залежно від  клінікопсихологічного профілю пацієнта. Описано потенціал цифрових терапевтичних методів (телематика, мобільні додатки, цифрова КПТ). Доведено, що комбіновані програми (фізична терапія разом з психологічними методами) суттєво покращують больові пороги та зменшують центральну сенситизацію.

Окреслено прогалини досліджень: потребу у  визначенні оптимальної «дози» психотерапії, вивченні довгострокових результатів та  фенотипуванні пацієнтів. Інтеграція психологічних методів є неодмінним компонентом ефективного ведення хворих із ХСМБ.
