ПРОБЛЕМА ДИАГНОСТИКИ И ТЕРАПИИ АУТОИММУННЫХ ЭНЦЕФАЛИТОВ

Анотація

Аутоімунні енцефаліти (АЕ) — це неврологічне захворювання, що характеризується ураженням переважно сірої речовини, в патоґенезі якого основну роль відіграють аутоантитіла до внутрішньо- або позаклітинних структур нервової системи, які виступають як антигени. Діагностичний алгоритм, поряд з проведенням комплексного обстеження, що включає нейровізуалізацію, електроенцефалографію, дослідження цереброспінальної рідини і визначення антитіл до NMDA-рецепторів, ще треба доповнювати, в деяких випадках, пошуком і онкологічного вогнища. Терапевтичний алгоритм АЕ передбачає комбіноване призначення глюкокортикостероїдної терапії, плазмаферезу, імуноглобулінів, цитостатиків, а також — при виявленні в деяких випадках онкологічного процесу — оперативне лікування.
