Abstract The purpose of research is to develop the new approaches due medical and social rehabilitation of especially socially dangerous schizophrenic patients. A cross-sectional, case series, case control study of 511 men population with paranoid schizophrenia, who committed grave and especially grave social dangerous act (SDA) were conducted. All patients were segregated into comparison groups subject to delict's psychopathological mechanism: the study included 251 patients with productive-psychotic mechanism SDA (I group) and 127 patients with negative-personal mechanism SDA (II group). The necessity of an integrated differentiated systematic assessing the overall criminogenic status specifi ed contingent within the triune (bio-psycho-social) approach — clinical and biological, psychological and social components, — was proved. The key objectives of the medical and psychological rehabilitation during compulsory treatment is a diagnostic the criminogenic factors, which involved in the formation of illegal behavior with subsequent elimination in accordance with the mechanism SDA. It is established that the value of social criminogenic factors are signifi cant, but smaller in the equation of clinical and personal. So the use the psychosocial rehabilitation approaches due to treatment and rehabilitation of such patients is insuffi cient to reduce their public danger.
