Abstract The literature review has shown the modern views on the complex treatment of cognitive disorders in patients after brain injury from the perspective of the hypothesis of “neuronal plasticity”. In terms of basic strategic approaches to the restoration of disturbed cognitive functions special attention has been paid to cognitive rehabilitation, as well as the problem of the choice of medication. The main purpose of neuroprotective therapy in patients after traumatic brain injury have been determinated. Modern place of nootropic and anticholineste rase medications in the treatment of organic mental disorders has been shown. The prospects of application of NDMA-antagonists and blockade of amyloidogenesis in the pathogenesis of cognitive dysfunction have been assessed.
