Abstract The coexistence of migraine and epilepsy, two widespread neurological diseases, remains an unresolved problem. The presented data concern the investigation of 168 patients with epilepsy of diff erent etiology (75 men and 93 women) aged 8—83 (mean age — 36.8 ± 8.4), who have had headaches of diff erent types in the course of the disease. The primary headaches of migraine type (that have been presented as pre-, inter- and post-ictal headaches) were established in 11.9 % patients. Migraine has been stated in patients with epilepsy of diff erent etiology, but for patients with idiopathic epilepsy migraine was diagnosed in 80 % cases. It could be indicative for probable common genetic mechanisms of the development of these diseases. In general, primary headaches were diagnosed in 42.8 % of cases and were observed more often in patients with cryptogenic and idiopathic epilepsy, and secondary — in symptomatic one (49.5 %). Migraine in symptomatic epilepsy occurred rarely (8.6 % of inter-ictal and 9.6 % of post-ictal headaches). At the same time secondary headaches in symptomatic epilepsy, which often are similar to migraine in clinical signs, mostly have been stated in patients with vascular, posttraumatic epilepsy, in space-occupying lesions of the brain or consequences of their removal. In these cases the pathophysiological mechanisms of headaches could be vascular (dilation or spasm of arteries), disturbances of cerebrospinal fl uid circulation, or mixed, that requires the corresponding diff erentiated treatment. The type of headache did not depend on duration of epilepsy.
