Abstract The aim is to determine the diagnostic value of serum concentration of vascular endothelial factor of height 1 (VEGF-1) for prediction of functional output in early recovery period of cerebral ischemic hemispheric stroke (CIHS). Complex clinical instrumental-laboratory research was done among 75 patients (the average age of patients is 57.2 ± 1.2 years) in early recovery period of CHIS using National Institute of Health Stroke Scale (NIHSS), Barthel Index, modifi ed Rankin Scale on the 10th, 30th, 90th and 180th day of disease, computed tomography of the brain. The level of VEGF-1 in blood serum was determined on the 10th and 30th day of disease, rate of ΔVEGF-1 refl ected the dynamic of given ctiteria was calculated. ROC-analysis was used for the development of criteria as for prediction. The patients with CIHS, whose indexes as for NIHSS Scale on the 180th day of CIHS were 4 points and more, on the 10th day diff ered by lower meaning of criteria ΔVEGF-1 in comparison with the patients with indexes as for NIHSS Scale on the 180th day of CIHS were less than 4 points (–034 [–0,51; –0,28] against –0,20 [–0,39; –0,11], p = 0.009). Decreasing of VEGF-1 from the 10th till 30th day of CHIS on more than 23.2 % is the predictor of value as for NIHSS Scale 4 points and more on the 180th day of disease (AUC = 0,74, p < 0,05; sensitivity — 92.3 %, spe cifi ty — 54.3 %).
