Abstract On the basis of long-term (mean 8.5 ± 3.0 years) prospective study of 404 patients with epilepsy of various etiologies (with an average age 36.8 ± 8.4 years) the structure and frequency of acute and chronic somatic pathology, psychiatric disorders, types of headaches has been studied, as well as the impact of these comorbid conditions on the course, clinical and therapeutic pathomorphosis of epilepsy. Random chronic somatic comorbidity, etiologically and pathogenetically not associated with epilepsy, number of concomitant somatic diseases, acute infections and surgical interventions did not aff ect the remote prognosis, clinical and therapeutic pathomorphosis of epilepsy. Pathology of the gastrointestinal tract and hepatobiliary system was associated with the development of pharmacoresistance to treatment, causing negative long-term therapeutic pathomorphosis of epilepsy, regardless of its etiology. Occurrence of somatic comorbidity as a complication of antiepileptic therapy testifi ed about negative health-related variant of therapeutic pathomorphosis of epilepsy. The presence of somatic diseases led to an increase of depression and anxiety symptoms, especially in patients with symptomatic epilepsy. Concomitant headaches of secondary type arose significantly more often in patients with somatic disorders, especially in drug-resistant epilepsy course.
