Abstract Epilepsy and seizures after neurosurgery is one of the most frequent complications that entail a number of problems related to the diagnosis, prognosis and treatment of these patients. No unambiguous criteria for risk of reactivation (change) seizures after epilepsy surgery is not defi ned recommendations for dose adjustments of antiepileptic drugs (AEDs) or possible cancellation. Most often relapse observed in patients with brain structural abnormalities which were determined in magnetic resonance imaging preoperatively. "Early" and "late" seizures de novo after neurosurgical operations have a diff erent mechanism of development and require a differentiated approach to the treatment. The development of seizures depends on the primary pathology and localization process, the type of surgery, the volume of the damage. The risk of seizures is reduced in preventing post-operative complications such as infection and hemorrhage. PEP treatment is applied in accordance with the basic principles of antiepileptic therapy, taking into account a number of factors (side effects, drug interactions, concomitant therapy, etc.). Question — the use of AEDs in order to prevent — currently remains controversial. Prophylactic use of AEDs eff ective against "early" seizures and ineffi cient for the "late" seizures.
